Technical Assistance:
Injury reports are to contain the following components: date and time of injury; name of injured child; type and circumstance of the injury; staff present and treatment; date and time when parents were notified; any future action to prevent recurrence of the injury; staff and parent signature or two staff signatures; and documentation on how parent was notified. No item should be left blank.
Over-the-counter skin products, such as diaper ointment, must be disposed of or sent home if expired.

Comments:
A monitoring inspection was initiated on 11/01/2023 and concluded on 11/01/2023 from 10:10 AM to 2:00 PM. There were 87 children present, ranging in ages from three months to five, with 20 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of eight children?s records and the eight staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.035-B-4
Description: (REPEAT VIOLATION) Based on record review and interview, the center failed to obtain out-of-state criminal history background checks for a staff member prior to the first date of employment.

Evidence:

1. The record for staff 1 contains documentation of a criminal history background check being processed for another state. The results were not in the file. The hire state for staff 1 was 10/11/23 and the start date was 10/16/23.
2. The administrator stated they have not received the results.

Plan of Correction: From now on out-of-state criminal background checks will be completed prior to new staff starting.

Standard #: 8VAC20-780-160-A
Description: Based on record review and interview the center failed to ensure staff submitted documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children that was completed within the last 30 calendar days of the date of employment.

Evidence:

1. The record for staff 2 documents the start date as 10/4/23. The TB documentation is dated 10/16/23.
2. The record for staff 4 documents the start date as 11/3/22. The TB documentation is dated 12/7/22.
3. The record for staff 2 documents the start date as 2/17/23. The TB documentation is dated 2/22/23.

Plan of Correction: From now on no staff will begin working until TB results are received.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the center did not follow hand washing requirements.

Evidence:

1. Staff 9 was observed changing diapers three children's diapers. Staff 9 did not wash any of the three children's hands after diapering.
2. Staff 9 did not wash her hands between each diapering.

Plan of Correction: All staff will be retrained in diapering standards.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center failed to ensure the diapering surface was cleaned with soap and at least room temperature water and sanitized after each use.

Evidence:

Staff 9 was observed changing three children's diapers. Staff 9 did not clean and sanitize the diapering surface between each diaper changing.

Plan of Correction: All staff will be retrained in diapering standards.
