Comments:
A monitoring inspection was conducted on November 9, 2023 with the center director and staff. There were 13 school age children present, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 2 staff records were reviewed. The children were eating dinner, playing outdoors, and working on homework.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-770-60-B
Description: Based on review of two staff files, the facility failed to have a completed sworn disclosure statement for each staff at the first day of employment. Evidence: Staff A (date of hire 10/27/2023) signed a sworn disclosure statement form on 9/22/2023, but did not mark an answer to the four questions on the form.

Plan of Correction: Staff 1 was asked to complete a sworn disclosure statement.

Standard #: 8VAC20-780-130-A
Description: Based on review of five children?s records, the facility failed to have the immunization record for each child before the child can attend the center. Evidence: Child 1 (date of enrollment 9/29/2023) and Child 2 (date of enrollment 8/29/2023) did not have documentation of an immunization record on file.

Plan of Correction: Center staff will request the immunization record from the parent. Some immunization records were deleted from the electronic system.

Standard #: 8VAC20-780-60-A
Description: Based on review of five children's files, the facility failed to have required information in each child's file. Evidence: Child 1 and Child 4 did not have documentation of the name, address, and phone number of two (non-parent) designated people to contact in an emergency if a parent cannot be reached. Child 1 and Child 4 only had parents/legal guardians listed.

Plan of Correction: Children's files will be audited for missing items.
