Comments:
An unannounced non mandate monitoring inspection was conducted on November 16 2023, from 9:35am-10:50am to follow up on the violations from the previous inspection. . At the time of entrance, ( 4 ) children were in care with ( 2 ) staff members present in room #108, one being the back up program director. The inspector reviewed 4 children?s records and 1 staff records. Children were observed during painting, manipulative, diapering and outdoor play time. No medications this date. Areas of non-compliance are identified on the violation notice. Repeat violation was found and is identified within this report.
Discussed concerns with the staff not speaking English and the 2 infants being in the same area with the 2 older children.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.


The facility is reminded that a complete renewal application must be submitted prior to expiration of the license on February, 6 2024. Renewal applications should be submitted through the online portal available on the public website at childcareva.com.

Description: Based on staff record review, one staff does not have a central registry finding within 30 days of employment.
Evidence: Staff #1, date of employment 10/16/2023 does not have documentation of completion of central registry results within 30 days of employment.

Description: Based on observation and staff and director interview, staff who work directly with children is not capable of communicating with emergency personnel.
Evidence: The inspector asked staff #1 the age of her daughter, she could not respond. She looked at the director and the director answered the question. The inspector asked the director if she could speak English, he responded, "her English is very limited".
Later the staff walked into an area where I was reviewing records to get a bottle. I asked ,who is the bottle for? She responded, "no English" I asked if she understood my question, her response was "no English".

Description: Based on observation, infants are not protected from older children.
Evidence: There were 4 children in room # 108, 2 were infants, the other children ages 3 and 4 years old. The infants were not protected from the older children. At one point staff placed 1 infant in a play yard but the 2 other children climbed in the play yard with the infant.

Standard #: 8VAC20-780-500-A
Description: Based on observation, one staff did not wash their hands with soap and running water before and after a diaper change.
Evidence: Staff #1 was observed changing infant # 2's diaper. She did not wash her hands after changing the infants diaper. She preceded to pick up another infant place them in a high chair then picked up puzzles pieces from the table, all without washing her hands.

Description: Based on observation, staff did not clean and sanitize the changing table after each use.
Evidence: Staff #1 was observed changing infant # 2, afterwards the staff did not clean and sanitize the changing table.

Description: Repeat Violation
Based on observation and staff record review, there was not at least two staff members who meet the requirements of current CPR and first aid certification on the premises during the center's hours of operation and wherever children are in care.
Evidence: The inspector arrived at 9:35 am, there was 1 staff who could not speak English and the substitute Director caring for 4 children. The substitute director was the only staff that had current CPR and first aid certification.

Standard #: 8VAC20-780-570-E
Description: Based on observation, prepared infant formula is not dated and labeled with the child's name.
Evidence: The bottle for child # 2 was not dated and did not have her name on the bottle, her initial was written on the top of the bottle. There were 2 infants, 1 being the staff's infant in care this day.
