An unannounced on-site monitoring inspection was conducted on 11/16/23 from 2:30pm until 3:15pm. At the time of entrance, there were 43 children in care with 3 staff present. Children were observed eating snack, completing homework and engaging in free play. Restroom and handwashing procedures and parent pick-up procedures were also observed. A sample of 5 children's records were reviewed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports, medication and required center postings were also reviewed.
A sample of 4 staff records were reviewed at the Greenbrier North YMCA on 11/16/23 from 10:15am until 11:00am.
Information gathered during the inspection determined non-compliances with applicable standards or law. One violation is documented on the violation notice issued to the program and discussed with center administration at the conclusion of the inspection.

Standard #: 22.1-289.035-A Description: Based on record review and interview, the center did not ensure that the results of a fingerprint based background check are obtained at least every five years for all staff.
Evidence:
1. The most recent fingerprint based background check for staff 1 is dated 9/20/18. Staff 1 was due for an updated fingerprint based background check in September 2023.
2. Center administration confirmed that the fingerprint based background check for staff 1 is expired.Plan of Correction: The center responded with the following: Staff 1 has an appointment to submit his fingerprints for an updated background check on 11/18/23. Once obtained, the results will be forwarded to the licensing inspector. Going forward, center administration will ensure updated background checks are obtained within the required time frames.
