Comments:
An unannounced monitoring inspection was initiated in the center On November 8, 2023 from 10:25 AM to 2:25 PM and concluded on November 21, 2023 regarding an inspection related to an Intensive Plan of Correction (IPOC) regarding the areas of Programs, Special Care Provisions/Emergencies and Special Services. There were observations of diapering, feeding and child to staff interactions. During the inspection the following were completed: interviews with administrative staff, center staff , a review of records/documentation and a review of centers vide recording.

The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. An additional areas of non compliance, not related to the IPOC was cited in the area of Administration and Physical Plant.

Standard #: 8VAC20-780-40-E
Description: Based on observation, staff interviews and review of center documentation, the licensee did not ensure they followed the operational responsibilities of the licensee to include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Evidence: The center did not follow their own safe sleep policy/procedure as their policy states that "A firm surface, such as a mattress will be used for infant sleeping. The mattress will fit snugly to the crib and will be covered with a fitted sheet". Two infants observed each sleeping in a crib and their sheets were not fitted to the mattress and had come loose and were bunched up under and around the infants in the crib.

Plan of Correction: Cot Sheets were removed from the infant room and replaced with crib sheets.

Standard #: 8VAC20-780-280-B
Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the following areas hazardous chemicals were observed, and they were labeled "keep out of reach of children" and at least one other statement "caution", "flammable" and "warning"
1.In the Infant 2 room, #109, there was an unlocked cabinet with containers of air freshener, hand sanitizers and disinfectant spray.
2.In the hallway near the fellowship hall and the children restroom, the pantry door was open and the floor there were gallon jugs of cleaner
3. The centers kitchen window/ counter was open and accessible to children as it was directly across from the two year old?s diapering station and there were spray bottles with disinfectant and fresher sitting on the counter.

Plan of Correction: All hazardous chemicals were locked away.

Standard #: 8VAC20-780-500-B
Description: Repeat Violation
Based on observation and staff interviews, the licensee did not ensure the diapering area had all required items and staff followed diapering procedures for handwashing.

Evidence: Staff #1 was observed conducting diaper change for a two year old in the fellowship hall and the diaper pad was not sanitized as required. Staff #1 completed their diaper change, they spayed the diaper pad with a sanitizing solution and then immediately wiped down the diaper pad. The diaper pad was not sanitized as it was not allowed to air dry or be wiped down only after 2 minutes.

Plan of Correction: Staff will wait 2min after sanitizing to change another diaper.
