An unannounced non-mandated monitoring focus inspection was conducted on November 20, 2023 from 7:12am-8:51am. At the time of entrance, approximatly 20 children were in care with 4 staff members present. Children were leaving on buses to go to school on both public school buses and YMCA transportation. The sample size consisted of 5 children?s records and 8 staff records.

Standard #: 8VAC20-780-130-A Description: REPEAT VIOLATION
Based on record review and staff interview, the licensee did not ensure the center obtain documentaiton that each child has recieved the immunizations required by the state board of health before the child can attend the center.
Evidence:
Child #4 start date 08/28/2023 and Child #5 start date 08/21/2023 did not have a documentation of immunization in there record as of November 21, 2023. Confirmed by staff.Plan of Correction: The immunization records for child #4 will be obtained.
Standard #: 8VAC20-780-140-A Description: REPEAT VIOLATION
Based on record review and staff interview, the licensee did not ensure each child have a physical examiniation documentation before the child's attendance; or within 30 days after the first day of attendance.
Evidence:
There was no documentation of physical for child #4 (start date 08/28/2023) and child #5 (start date 08/21/2023) with in 30days after the first day of attendance. Confirmed by staff.Plan of Correction: Child #4 will obtain their physical.
Standard #: 8VAC20-780-160-A Description: REPEAT VIOLATION/SYSTEMIC DEFICIENCY
Based on record review and staff interview, the licensee did not ensure each staff prior to employment shall have documentation of negative tuberculosis test.
Evidence:
The center was unable to provide documentation of a negative tuberculosis screening for Staff #4.Plan of Correction: Staff will obtain a TB screening by 12/31/2023.
Standard #: 8VAC20-780-70 Description: REPEAT VIOLATION
Based on record review, the licensee did not ensure to obtain name, address, and telephone number of a person to be notified in an emergency for all staff and which shall be kept at the center.
Evidence:
The center did not have emergency contact information with name , address and phone number for staff #8. Confirmed by staff.Plan of Correction: Staff #8 will complete an emergency contact info sheet for the St. Lukes location.
