Comments:

A monitoring inspection was conducted on 11/14/2023 from approximately 12:15 p.m. to 5:15 p.m. There were 32 children present ranging from infants to school age and 8 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child (5) records and fourteen (14) staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description:	Based on staff record review, the provider failed to obtain the results of a criminal history record information search, a sex offender registry check and out-of-state search of the child abuse and neglect registry from any state in which the individual has lived in the preceding five years. Evidence: Staff record for S11 documented on a resume having lived out-of-state in the last five years . Record for S11, employed 08/28/2023 did not contain a sex offender registry check, out-of- state child abuse and neglect registry search, or criminal history record information check for that state.
Plan of Correction:	Staff member on leave until all background checks are completed.

Description:	Based on observation, the provider failed to ensure that children in the Infant B classroom had rest mats linens consisting of a top cover and a bottom cover. Evidence: Three rest mats used by children during the rest period did not have a bottom covering.
Plan of Correction:	Get the proper bottom bedding for these mats.

Description:	Based on observation, the provider failed to ensure prepared infant formula shall be dated. Evidence: Individual infant bottles for child, C5 observed in the infant room refrigerator were not dated.
