Please ensure training hour logs are current with hours and certificates attached. First Aid and CPR can be counted for the full training hours as well as MAT but not Daily Health Observation as that is a separate requirement.

Comments:
An unannounced monitoring visit was conducted today. Nine classrooms were in operation with direct care staff in ratio. Observed children taking nap in the classrooms. Classrooms are well equipped and maintained with materials and equipment. Reviewed a sample of staff and children's files, medication on-site, trainings, emergency preparation and nutrition (lunch served today was meatball subs, fruit and salad). Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please be aware that the plan of correction must be provided within 5 days of receipt of the inspection notes if you want it displayed on the website. Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, a staff member's background check has not been repeated every five years.
Evidence:
Staff #1's Central Registry Search (CPS) expired 11/2022 and a new CPS clearance is not available.

Plan of Correction: The CPS form already submitted for search, HR waiting for the result.

Standard #: 8VAC20-780-60-A-7
Description: Based on record review, an allergy action plan was not complete as the medication was not available.
Evidence:
A medication listed by the physician was not on-site for Child #1.

Plan of Correction: The medication was removed by the doctor from the allergy plan. Child needs only the Epi-pen.

Standard #: 8VAC20-780-340-A
Description: Based on observation, a staff member did not ensure care, protection and guidance at all times of a child in care.
Evidence:
Staff #2 was unable to ensure care of Child #1 during rest time as the child was awake and alone in the room and the only staff member had fallen asleep.

Plan of Correction: Center manager met with the teacher and discussed the incident and the importance of children supervision. Center managers will walk the hallways to visit each classroom during naptime between 1:00-2:30 pm to ensure the children are supervised.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the center failed to keep a child under 10 years of age within actual sight and sound supervision.
Evidence:
Child #2 was in the classroom with Staff #2 who was observed asleep, facing away from the child for approximately 2-3 minutes.

Plan of Correction: Center manager met with the teacher and discussed the incident and the importance of children supervision. Center managers will walk the hallways to visit each classroom during naptime between 1:00-2:30 pm to ensure the children are supervised.
