Comments:
An unannounced monitoring inspection was initiated on November 30, 2023 and concluded on December 5, 2023. There were 43 children present, with 9 staff supervising. The inspector observed the children playing with toys and interacting with staff, completing a craft, handwashing, and preparing for lunch. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Staff records were reviewed remotely.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Description: Based on record review, the center failed to ensure that all staff complete a sworn statement prior to the first day of employment.

Evidence:
The record for staff #2 did not contain a sworn statement. Staff #2?s date of hire was 1/3/2023.

Description: (Repeat Violation) Based on review of records and interview, the center failed to provide an annual health approval from the health department.

Evidence:
1. The most recent health inspection was dated 5/23/2022.
2. An administrator confirmed that an email had been sent to the health department on 9/28/2023 regarding the health inspection.

Standard #: 8VAC20-780-510-P
Description: Based on review of medication, the center failed to ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

Evidence:
1. Child #1 had a prescription emergency medication, on site that expired 10/2024. The medication had not been administered.
2. Staff #2 was aware that the medication had expired.
