Comments:
An unannounced non-mandated focused monitoring inspection was conducted on 11/21/2023 from 9:30 am to 11:47 am. The Licensing Inspector reviewed three children?s records, two medications and associated records, four staff records, and other program records. The facility management has requested a training session on record keeping.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Based on document review, the center failed to ensure staff had a TB screening every two years.

EVIDENCE:
The most recent TB screening for staff 4 was dated 9/15/2021.

Plan of Correction: The staff person will have a TB screening right away, and all records will be checked to make sure they are done on time from now on.

Standard #: 8VAC20-780-80-A
Description: Based on document review, observation, and interview, the facility failed to maintain a written record of daily attendance that documents the arrival and departure of each child in case as it occurs.

EVIDENCE:
The inspector counted 16 children in care in the Maple room. The attendance sheet showed 15 children were in care. A staff person in the classroom stated they didn?t update the attendance sheet when a child came in earlier.

Plan of Correction: The attendance sheet was corrected immediately, and from now on staff will record arrivals and departures of children as it happens.

Standard #: 8VAC20-780-510-G
Description: REPEAT VIOLATION

Based on observation, the center failed to ensure that medication for a child was labeled with all required elements.

EVIDENCE:
Two prescription medications, a Novolog insulin pen, and Baqsimi, were in a zipper bag. Neither medication was labeled with a name, the dosage amount, and the time or times to be given. Staff 1 identified both medications as belonging to child 1.

Plan of Correction: The medication person at the center contacted the parents immediately, and they will bring in the prescription labeled packaging. From now on, all medications will be checked for labeling monthly.
