An unannounced inspection to investigate a self-reported incident regarding supervision was initiated on 11/13/2023 beginning at 10:45 a.m. to 11:45 .am.
Staff interviews were conducted. Video footage was reviewed. Documentation was reviewed. Observation of the classroom was conducted.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.
Please provide plan of correction for the area of noncompliance and date to be corrected by 12/13/2023.

Standard #: 8VAC20-780-340-F Description: REPEAT VIOLATION
Based on staff interviews and review of video footage, it was determined that the center failed to ensure that children under the age of 10 years of age were always within actual sight and sound supervision.
Evidence: On 11/02/2023, Child #1, 3 years of age, was left alone in their classroom from 11:39 a.m. to 11:41 a.m., approximately 3 minutes.Plan of Correction: Not available online. Contact Inspector for more information.
