Comments:
An unannounced monitoring inspection was conducted on 12/11/2023 from 11:20 a.m. to 12:40 p.m. There were 33 children present, ranging in ages from two to five years old with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation and interview the center failed to maintain the required staff: children ratio of 1:10 for children ages three years old to school eligibility.
Evidence:
1. In the PreK classroom, there were 12 children. Staff #6 confirmed the age of the youngest child was four years old.
2. Staff #7 was observed leaving the classroom twice to go to the kitchen, leaving Staff #6 alone with 12 children.

Plan of Correction: Talk to (Staff #7) about the importance of staying in her room so the other teacher is not left alone throwing her out of ratio.
