Comments:
A monitoring inspection was conducted on 11/30/2023 from approximately 2:35 p.m. to 5:25 p.m. There were 59 school age children present and 5 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Eight child (8) records and five (5) staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return within 5 business days. Your plan of correction should contain: 1)
steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on observation, the provider failed to ensure that any vehicle used by the center for transportation of children shall be kept in a satisfactory condition to assure the safety of children.
Evidence:
Licensing inspector observed a large crack across the entire front window shield of the bus (Virginia plate VRJ-6708) used to transport children enrolled in the after-school program at the center.

Description: Based on observation and interview, the provider failed to ensure that a list of the names of the children being transported were in the vehicle.

Evidence:
Licensing inspector interviewed S4 (bus driver) and requested see the notebook and the child transportation sheets used by the YMCA document the list of children being transported to the after-school program. She reported not having the list in the bus notebook and was working from a picture on her phone. The program director reported that list of children to be transported is updated weekly and that it the policy to use a paper list to document this information.
