A monitoring inspection was conducted on 12/05/2023 between the hours of approximately 1:00 pm and 3:00 pm. There were 27 children present, ranging in ages from 3 to 5 years old, with 6 staff supervising. Children were observed during their scheduled rest time.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 6 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-H Description: Based on documentation review, the center did not provide to the licensing representative evidence of public liability insurance coverage.
Evidence:
1) On the date of inspection, the most recent proof insurance document available for review was dated 09/29/2022-2023.Plan of Correction: Information was obtained from insurance company.
Standard #: 8VAC20-780-60-A Description: Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #2 did not contain the address of two designated people to call in an emergency if a parent cannot be reached.
2) The record for Child #4 did not contain the work phone number of each parent who has custody.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-420-E-3 Description: Based on record review, the provider did not ensure to annually, review with the parent the emergency contact information required to
ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
