Discussed in detail the requirements related to Playground Equipment-Resilient Surfacing (8VAC20-780-330) regarding spring rockers and merry- go-cycles.

An unannounced monitoring inspection was started in the center on December 14, 2023 from 12:20 PM until 2:35 PM and concluded on December 14, 2023. At the time of entrance, 9 children were in care with 3 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during their rest period and an infant was playing.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Repeat Violation
Based on record review and staff interview the licensee did not ensure documentation of the Tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: Staff #4 started employment with children on 10-2-2023 and TB screening was dated 10-13-23Plan of Correction: All newly hired employees will have documented TB before contact w/ children
Standard #: 8VAC20-780-160-C Description: Based on record review and staff interviews, the licensee did not ensure that each staff member had obtained at least every two years from the date of the initial screening or testing, the results of a follow- up tuberculosis screening.
Evidence: The records for Staff #1 did not include updated TB screening as the most recent screening for was dated 3-16-2021.Plan of Correction: Staff #1 will get updated TB & document will be in file
Standard #: 8VAC20-780-450-A Description: Based on observation and staff interview, the licensee did not ensure cots/mats used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidence: In the Infant Room 2 out of 2 children sleeping on a mat did not have a bottom cover or a top cover.Plan of Correction: All children will have a bottom cover and a top cover.
