Comments:
A mandated monitoring inspection was conducted on December 13, 2023 from 11:10 A.M.-12:30 P.M., and concluded on December 14, 2023 after a desk review of staff records. There were 16 children present, ranging in ages from eight months to three years of age, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: REPEAT

Based on review of staff records, the center failed to ensure that documentation of a tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children.

Evidence:
1. Staff #1's date of hire was 11/09/2023. The tuberculosis documentation in the record was dated 11/17/2023.
2. Administrative staff verified the date of employment and the date on the tuberculosis documentation.

Plan of Correction: In the future before an employee starts administration will verify that the correct tuberculosis paperwork is on file.

Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials be kept in a locked location.

Evidence:
In the restroom in Classroom #1 there was a bottle of bleach water in the cabinet in the restroom. The cabinet had a lock on it, but it was not locked.

Plan of Correction: Staff locked the cabinet while the Licensing Inspector was on site. Staff will be reminded to lock all cabinets after use when they have hazardous materials stored in them.

Standard #: 8VAC20-780-520-C
Description: Based on observation, the center failed to ensure that diaper ointment was labeled with the child's name.

Evidence:
1. In Classroom #2 there was a tube of Aquophor Healing Paste that was not labeled with the child's name.
2. Staff #2 verified that it was not labeled with the child's name.

Plan of Correction: The diaper ointment was removed from the room. In the future as diaper ointment is brought in staff will ensure that it is labeled with the child's name. Staff will do monthly audits on all over the counter products to ensure they are labeled and within date.
