Comments:
A mandated monitoring inspection was conducted on December 12, 2023 from 3:40 P.M.-4:40 P.M. on site and concluded on 12/13/2023 at administrative offices for staff record review. There were 12 children present, ranging in ages from five to nine years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on a review of staff records, the center failed to ensure that documentation of a tuberculosis screening was within the last 30 days of the date of employment.

Evidence:
1. Staff #1's start date was 10/02/2023. The tuberculosis screening documentation in the record was dated 07/03/2022.
2. Administrative staff verified the date of the employment and date on the tuberculosis screening.

Plan of Correction: In the future when new employees are hired tuberculosis documentation will be verified to ensure that it is within the correct timeframe.

Standard #: 8VAC20-780-190-B
Description: Based on review of staff files, the center failed to ensure that program directors without management experience shall have one college course in a business related filed, 10 hours of management training, or one child care management course.

Evidence:
1. Staff #2's file listed the job title as Site Director. There was no documentation of management experience in Staff #2's record.
2. Administrative staff verified there was no documentation of management experience in the record.

Plan of Correction: The director has been given a management course to complete. Administrative staff will ensure that it completed before she comes back to work.
