Technical Assistance:

Cribs versus cots/mats for infants over twelve months of age was discussed with the administrators. Cribs may be used after twelve months for infants if it is determined that the infant is not yet developmentally ready to be placed on a cot.

Comments:

A renewal inspection was conducted on 12/14/2023. The inspector arrived at 11:10 AM and departed at 2:00 PM. There were 130 children present with 25 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 13 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were reviewed with the administrators during the exit interview.

Description:	Based upon review of staff records, the facility has not ensured that each staff record includes the name, address and telephone number for a person to be contacted in an emergency. Evidence: The record provided for staff 11 did not include the address of the listed emergency contact person.
Plan of Correction:	The facility responded with the following: We corrected the 1 staff member that was missing an emergency contact address; we checked all other staff tiles to ensure all informaiton was complete, which was the case.

Description:	Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children. Evidence: 1. There was a spray bottle labeled bleach/water in an unlocked cabinet under the diaper changing table in Room 4. No diapers were being changed at the time. 2. Staff 2 verified that the bleach water was not in a locked location.
Plan of Correction:	The facility responded with the following: The teacher locked the cabinet immediately; We retrained all staff on having cabinets locked at all times.

Description:	Based upon observation and staff interview, the facility has not ensured that tables are sanitized after being used for eating. Evidence: 1.The children in room 9 were observed finishing lunch at the tables. The children washed their hands, then sat back down at their places at the table where they were given books to look at. The tables were not sanitized after lunch and before the children sat back down with the books. 2. Staff 1 observed that the tables still had spills from lunch on the tables when children sat back down with books.
Plan of Correction:	The facility responded with the following: We retrained all staff on ensuring sanitation throughout the day.
