Comments:
An investigation was initiated on 9/14/23 in response to a complaint received by the department on that same day regarding concerns in the areas of supervision and children's injuries. The complaint was investigated jointly with the local agency. An on-site inspection was completed on 9/20/23. Interviews with staff were conducted, a tour of the entire building and outdoor play area was completed and center documents related to the complaint were reviewed. Based on the information gathered during the investigation, the complaint is determined to be valid. Violations are cited on the violation notice and were discussed with center management at the conclusion of the investigation.

Description: Based on staff interviews, the center did not ensure that parents shall be notified of any known minor injuries by the end of the day.

Evidence:
1. The parent of child 1 reported that after picking the child up from the center on 9/11/23 they observed what appeared to be bite marks on the child.
a. Child 1's parent stated that they were not notified of any injuries to the child when she was picked up from the center that evening and were only told that child 1 had been bitten by another student the following morning when the director was questioned about the marks to the child.
2. Staff 1 stated that on 9/11/23 she observed a red mark on child 1's leg after another child bit her. Staff 1 stated that she did not inform child 1's parent of the injury at pick-up that evening.
2. Staff 2 stated that she observed a red mark to child 1's leg on 9/11/23. Staff 2 confirmed that the parent was not notified of the injury until the following morning when the parent inquired as to how child 1 sustained the mark to her leg.

Plan of Correction: The center responded with the following: The director will do a retraining with all staff about writing OOPs reports at the time of the incident and on the same day of the incident. This includes all minor and major incidents. The parent/guardian must be notified on the same day of the written OOPs report and staff are reminded to receive a signature from the parent for verification of written notice provided. Staff must also notify the director at the time of the incident.

Description: Based on staff interview and a review of children's injury reports, it was determined that the center did not ensure that a written record of children's serious and minor injuries are maintained in which entries are made the day of the occurrence.

Evidence:
1. Staff 1 stated that child 1 was bit by another child on 9/11/23 but that she did not fill out an injury report for the incident until the following morning after child 1's parent inquired about the bite mark.
2. Staff 2 confirmed that an injury report for child 1 was not completed until the morning after the injury occurred.
