Comments:

An unannounced renewal inspection was conducted today, December 27, 2023. The inspector was on site from approximately 3:30 pm until 5:30 pm. The owner/ director was present and assisted with the inspection. There were 16 children present, ranging in ages from thirteen months to five years, with four staff supervising. Children were observed playing with toys and watching a children?s video. Dismisal procedures were also observed. The inspector interacted with staff and children in care. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #:	22.1-289.035-B-4
Description:	Based on review of six staff records and interview, the center did not request an Out of State Child Abuse and Neglect Search Request by the End of 30th Day of Employment, an Out of State Sex Offender Registry completed prior to the 1st day of employment and an out of state criminal history name check required for non-NFF states only, prior to the 1st day of employment from any state in which one staff member had resided in the preceding five years. Evidence: 1. The record of staff #2, employed 10/13/2023, indicated on the signed sworn statement, dated 10/09/2023, that staff #2 had resided outside the state of Virginia in the past five years. The record of staff #2 did not contain documentation of an out of state Child Abuse and Neglect Search, out of State Sex Offender Registry, nor an out of state criminal history name check as required. 2. The director stated an out of state Child Abuse and Neglect Search, out of State Sex Offender Registry, nor an out of state criminal history name check had been requested for staff #2.

Description:	Based on review of six staff records, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children for two staff members. The documentation shall have been completed within the last 30 calendar days of the date of employment. Evidence: 1. The record of staff #1, employed 12/14/2023, contained documentation of a tuberculosis screening dated 12/16/2023, exceeding the date of employment. 2. The record of staff #3, employed 11/02/2023, did not contain documentation of a tuberculosis screening. The director stated she did not have the documentation.

Description:	Based on review of four staff records and interview, the center did not obtain all required documentation for each staff record. Evidence: The record of staff #1 (employed 12/14/2023) and staff #3 (employed 11/02/2023) did not contain documentation of two references as to the character and reputation as well as competency. The record of staff #2 (employed 10/23/2023) contained one reference when two references are required to be obtained and documented before employment. The director stated references for the three staff could not be located.
