Comments:
A monitoring inspection was conducted on 12/29/2023. The inspector arrived at 12:00 PM and departed at 1:25 PM. There were 18 school age children present with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-60-A-8
Description: Based upon review of children's records and staff interview, the facility has not ensured that they have obtained a written care plan for each child with a diagnosed food allergy.
Evidence:
1. The record provided for child 7 included documentation of a food allergy for which Benadryl and an EpiPen were stored for emergency use. The record did not include a written allergy care/management plan.
2. Staff 1 verified that there was not a written care plan for child 7.

Plan of Correction: The facility responded with the following:
All children with diagnosed food allergies will have allergy management plans completed by a physician in the records.

Standard #: 8VAC20-780-260-A
Description: Based upon staff interview, the facility has not ensured that there is an annual fire inspection.
Evidence:
Staff 1 was unable to provided documentation of an annual fire inspection. The most recent documentation is dated 4/18/2022.

Plan of Correction: The facility responded with the following:
An annual fire inspection will be obtained and documentation will be placed in the record.

Standard #: 8VAC20-780-260-B
Description: Based upon staff interview, the facility has not ensured that there is an annual health department inspection.
Evidence:
Staff 1 was unable to provided documentation of an annual health department inspection. The most recent documentation is from 2022.

Plan of Correction: The facility responded with the following:
A health department sanitation inspection will be obtained and documentation will be placed in the records.

Standard #: 8VAC20-780-510-B
Description: Based upon review of medication documentation and staff interview, the facility has not ensured that there is written authorization from the parent for medication to be administered.
Evidence:
1. There is albuterol and an EpiPen for administration to child 6 in an emergency. There was no written parent authorization for administration of either medicine.
2. Staff 1 acknowledged that there was not written parental authorization for the albuterol and the EpiPen.

Plan of Correction: The facility responded with the following:
The parent will be required to complete a medication authorization form for both medications. In the future, parent authorization will be obtained for all medication brought in to potentially be administered to children.
