Comments:

A monitoring inspection was conducted on 12/15/2023. The inspector arrived at 9:15 AM and departed at 11:30 AM. There were 25 children present, ranging in ages from 2 yrs to school age, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Standard #:	22.1-289.035-B-4
Description:	Repeat violation Based upon review of staff records and staff interview, the facility has not ensured that results of a criminal history record check and a search of the child abuse and neglect registry have been obtained from any state in which the individual has resided in the preceding five years. Evidence: 1. The record provided for staff 5 indicated that staff 5 resided in Nevada within the past five years. Results of a criminal record check have not been obtained. 2.Staff 1 acknowledged that a criminal record check from Nevada has not been requested for staff 5.
Plan of Correction:	The facility responded with the following: A criminal record check will be immediately requested. In the future, we will ensure that results of criminal record checks from any state in which the applicants have lived in the preceding five years are obtained prior to employ.
Standard #:	8VAC20-780-160-C
Description:	Based upon review of staff records, the facility has not ensured that staff obtain and submit results of a tuberculosis screening at least every two years. Evidence: 1. The most recent tuberculosis screening in the record provided for staff 3 is dated 6/16/2021. 2. Staff 1 acknowledged that staff 3 has not submitted results of a tuberculosis screening within the past two years.
Plan of Correction:	The facility responded with the following: Staff 3 will obtain a tuberculosis screening as soon as possible. reminders regarding tuberculosis screenings will be tracked in the future.

Description:	Based upon review of children's records and staff interview, the facility has not ensured that children's records include the names, addresses and telephone numbers for two persons to be contacted if a parent cannot be reached. Evidence: 1.The record provided for child 4 did not include the street addresses for the persons to be contacted in an emergency if a parent cannot be reached. 2. The record for child 6 did not include the street address for one of the persons to be contacted in an emergency if a parent cannot be reached. 3. Staff 1 acknowledged the missing addresses listed above.
