Comments:

A renewal inspection was conducted on December 22, 2023, from approximately 7:50 am to 10:10 am. There were 19 children present in the center, ranging in age from two year old's to four year old's, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A member of management stated there were no medications on site and transportation is not provided. A total of five child records, five staff records, and two board member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description:	REPEAT VIOLATION Based on a review of staff records and interview, the center did not ensure that two staff had the required repeat background checks every 5 years. 1. The record for staff #5, did not have documentation of an updated fingerprint-based national criminal background check (FP: 2/20/18). An updated fingerprint-based national criminal background check was required by 2/20/23. Staff #5 had documentation of an updated Sworn Statement on 6/26/23. An updated Sworn Statement was required by 7/05/22.

Description:	Based on observation, the center did not ensure all hazardous substances such as cleaning materials insecticides and pesticides, were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the toddler classroom, one container of disinfectant spray, disinfecting wipes, and one container of pumpkin spice spray on the countertop.
Plan of Correction:	After cleaning in the evenings staff will be responsible for completing a walkthrough of class before leaving for the day.
