Comments:
An unannounced monitoring inspection related to a provisional license was started in the center on January 18, 2024 from 10:25 AM until 2:05 PM and concluded on January 24, 2024. At the time of entrance, 81 children were in care with 17 staff members and additional administrative staff were on site. Th, sample size consisted of 19 staff records. Children were observed during lunch, diapering, circle time and freely playing.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4
Description: Repeat Violation
Based on record review and staff interviews, the licensee did not ensure, all staff had obtained an out of state criminal history name check (CRC) prior to the first day of employment and an out of state child abuse and neglect search (CPS) was request by the 30th day of employment for any state where they lived within the last five years from date of hire (DOH).

Evidence:
Staff #10's record did not include an out of state CPS for North Carolina (DOH 10-30-2023).
Staff #17's record did not include an out of state CPS for North Carolina (DOH 7-31-2023).
Staff #19?s record did not include an out of state CRC for South Carolina prior to first day of employment (DHO 1-8-2024). South Carolina CRC results were dated 1-9-2024.

Plan of Correction: Staff #10 CPS check submitted to licensing and added to file
Staff #17 CPS check submitted to licensing and added to file
Staff #19 was not w/ children until 1/9. All CRC's will be done prior to hire or w/ children.

Standard #: 8VAC20-770-60-B
Description: Based on record review and staff interviews, the licensee did not ensure each staff member had a Sworn Statement completed prior to 1st day of employment.

Evidence: Staff #1?s Sworn Statement was dated 12-15-2023 which is after the 1st date of employment on 11-20-2023.

Plan of Correction: Stiff #1's original sworn statement dated 11/20/23 put in file.

Standard #: 8VAC20-780-280-B
Description: Repeat Violation
Based on observation, the licensee did not ensure hazardous substances and other harmful agents such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: The following items were observed stored in an unlocked manner and were accessible to children. The hazardous chemicals were labeled "keep out of reach of children" and at least one other statement "caution", "flammable" and "warning".
1. In the pantry closet across from the room #106, (pre-k 3), there was an unlocked closet and on the floor, there were 10 containers of dish detergent, 4 containers of disinfectant and 2 containers of floor cleaner.
2. In room #106 B, in an unlocked cabinet, 1 container of disinfectant and 2 mops with attached containers of disinfectant/cleaner were sitting in the corners of the room.
3. In room #108 B, in an unlocked cabinet, 2 containers of disinfectant, 2 containers of hand sanitizer were sitting on the counter and 1 container of hand soap near the sink.
4. In room #110 B, in 1 mop with attached containers of disinfectant/cleaner was sitting in the corner of the room.
5. In room #103 D, 1 mop with attached container of disinfectant/cleaner was sitting in the corner of the room, 1 container of hand sanitizer and 1 container of hand soap were near the sink.

Plan of Correction: 1. All hazardous substances removed from pantry.
2.The disinfectant was locked up and the Swiffer removed.
3.The hand sanitizer was removed. the Swiffer was removed. the disinfectant was locked up.
4. the Swiffer was removed.
5. the Swiffer was removed. The hand sanitizer was removed. this disinfectant was locked away.

Standard #: 8VAC20-780-530-A
Description: Based on record review and staff interviews, the licensee did not ensure there was at least one staff in each classroom or area where children are present had obtained a current certification in cardiopulmonary resuscitation (CPR) and First Aid as appropriate to the age of the children in care

Evidence: Staff #4 and Staff #19 were the only staff observed supervising 8 infants and Staff #4 and #19 did not have a certification in CPR and First Aid.

Plan of Correction: A CPR & First Aid certified staff member will swap until staff #4 and #19 are certified. CPR scheduled 1/30/24
