The licensing inspector conducted an self report in regards to outside medical treatment that was received by the licensing department on January 9, 2024 . The inspection was initiated on January 18, 2024. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-430-F Description: Based on observation, licensee did not ensure the climbing portions of indoor slides and climbing equipment over 18 inches shall not be over bare flooring.
Evidence:
In the School age area, there was climbing equipment the highest point was 2 feet. The equipment was over laminate floor.Plan of Correction: Black mats were ordered immeidatley January 10, 2024. And additional three packet of mats were order January 18, 2024 to cover all area around the equipment.
