The licensing inspector initiated an unannounced inspection on 01/11/2024 and concluded on 1/11/24 in response to a complaint received by the department on 12/29/2023 for allegations regarding supervision and staff training. Interviews and observations were conducted relating to the allegations. The information gathered during the investigation support the allegation in part, so the complaint is determined to be "not valid."
