Comments:
A complaint inspection was initiated on 1/25/2024 and concluded on 1/25/2024. There were 28 children present with six staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision. A total of two child records and six staff records were reviewed. The inspection started at 1:30 pm and concluded at 3:10 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Complaint related: No
Description: Based on record reviews the center failed to ensure that the child day center obtained a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
Staff #2 (hire date 2/1/23) and staff # 3 (hire date 11/30/23) listed on the sworn statement that they have resided or are residing in another state in the preceding five years. There are no background checks in staff files for the listed state.

Plan of Correction: Background checks are scheduled for staff #2 on 2.20.24 and staff #3 on 2.14.24.

Description: Based on evidence each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence:
Staff #1 hire date 11/29/23 has a TB screen dated 1/2/23; staff #2 hire date 2/1/23 has a TB screen dated 5/2/23; staff #3 hire date11/30/23 has a TB screen dated 1/5/24; staff #6 hire date has a TB screen dated 9/21/23. All of the staff listed did not receive their TB screening within the required timeframe.

Complaint related: Yes
Description: Based on record review and interviews the center failed to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence:
In July of 2023 a child was injured while at the center. This injury required outside medical treatment. The center did not inform the licensing inspector of this injury.

Plan of Correction: Center will notify Licensing Inspector shall such occurrence happen again.

Complaint related: No
Description: Based on record reviews and interviews the center failed to ensure that all staff within six months before being promoted or beginning work or one month after being promoted or beginning work, a minimum of 24 hours of training shall be received related to the care of children.
Evidence:
Staff #2 (hire date 2/1/23) and staff #3 (hire date 11/30/23) do not have documentation in their files that they have met the requirements to be a lead teacher. Both staff were working on the day of inspection in lead teacher positions.
