Comments:
A monitoring inspection was conducted on February 2, 2023 with the center director and staff. There were 23 children present, ranging in ages from 2 years to 3 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 staff records were reviewed. The children were having lunch and preparing for rest.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-770-60-B
Description: Based on review of three staff records, the facility failed to have documentation of a completed sworn disclosure statement on file by the first day of employment. Evidence: Staff 1 (date of employment 6/1/2023) did not complete a sworn disclosure statement until 6/07/2023. Staff 2 (date of employment 7/05/2023) did not have documentation of a sworn disclosure until 7/10/2023.

Plan of Correction: All staff will sign a sworn disclosure statement at the first day of employment.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of three staff records, the facility failed to have documentation of a completed search of the central registry (CPS check) on file for each staff within the first 30 days of employment. Evidence: Staff 1 (date of employment 6/1/2023), Staff 2 (date of employment 7/05/2023), and Staff 3 (date of employment 7/17/2023) did not have a completed search of the central registry on file. There was no documentation of having submitted the search of the central registry within 7 days of employment as required.

Plan of Correction: Director was having issues with the portal. Licensing inspector and director logged into the CPS portal during the inspection.

Standard #: 8VAC20-780-140-A
Description: Based on review of five children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 1 (date of enrollment 12/08/2023) did not have documentation of a physical.

Plan of Correction: Staff will request the physical from the parents.

Standard #: 8VAC20-780-160-A
Description: Based on review of 3 staff records, the center failed to obtain documentation of a negative tuberculosis (TB) screening or test within the required time frame. The test or screening must be done at the time of employment, prior to coming in contact with children, and no more than 30 days prior to of the date of employment. Evidence: Staff 2 (date of employment 7/5/2023) had a TB test 10/09/2023 and Staff 3 (date of employment 7/17/2023) had a TB test on 10/06/2023.

Plan of Correction: Staff will provide a TB test or screening within the required timeframes and prior to contact with children.

Standard #: 8VAC20-780-550-F
Description: Based on review of the emergency drill log and staff interview, the facility failed to conduct a lockdown drill at least once annually. Evidence: there was no documentation of a
lockdown drill. Staff confirmed that a lockdown has not been conducted.

Plan of Correction: A lockdown drill will be conducted this month.
