8VAC20-780 Staffing and Supervision

Comments:
A self-reported monitoring inspection was initiated on 01/19/24 and concluded on 01/26/24. The inspector reviewed compliance in the area of staffing and supervision.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-A
Description: Based on a self-report of two (2) incidents the facility failed to ensure the care , protection, and guidance of the children.

Evidence:
On 1/19/24 child #1, 6 years old, was witnessed by staff #1, who was in the classroom, exiting the classroom door onto the gated area outside of the classroom. Staff #1 was the only teacher in the classroom and stood with the door open to continue to supervise child #1 and also the children that were in the classroom. There were 7 children and 1 staff meeting ratio.
Child #1 then proceeded to attempt to exit the gated area that abutted the parking lot. At this time staff #1 closed the outside door and got another teacher to stand in the room while she went out onto the gated area to retrieve the child.
Statements from staff said that although the staff member closed the door to obtain assistance, the child was seen on video by front office staff who immediately went to observe the child to ensure the child's supervision, safety, and to assist.

On 1/25/24 at approximately 7:22am child #2, 2 year old, was seen standing in the hallway by the front door by staff member #4 who was arriving for her shift. Staff #2 and #3 were unaware that the child had left the classroom during hand washing time. Documents showed there were 12 children and 2 staff meeting ratio.
Staff reported that the child was in the area alone for no more than 30 seconds.

Plan of Correction: 1. Staff will be retrained on the Center Operations Quality Foundations and Supervision of Children section and sign off that they have received the training by 2-2-24.
2. Staff will also be required to complete the Safety Tools Active Supervision Training and sign off on completion by 2-2-24.
3. In addition to staff training, management will review staffing patterns and make any possible changes to provide additional staffing support.
4. The administration team will partner with facilities for them to install doorbell alerts on all interior doors leading from the classrooms to the hallways. These already are installed on the doors leading to the outside from the classrooms.
5. Finally, when developmentally appropriate, behavior guidance plans surrounding eloping behaviors will be made with caregivers including setting up plans to have children picked up from the center if there are repeat attempts.

Evidence:

Staff #4 took child #2 back to her classroom where staff #2 and #3 were found and unaware that the child had left the classroom during hand washing time.
