On 01/11/2024 and 01/25/2024 an unannounced inspection was conducted in response to a self-reported incident received on 12/18/2023. Interviews were conducted regarding the incident and records and policies and procedures were reviewed. The information gathered during the inspection resulted in violations. Areas of non-compliance are identified on this Violation Notice.

Standard #: 8VAC20-780-40-E Description: Based on interview and a review of the parent handbook, the licensee failed to ensure that the center is maintained in compliance with the center?s own policies and procedures that are required by the standards.
Evidence:
1. The center?s discipline policy noted in the parent handbook states the following: No discipline techniques shall be harmful or hurtful towards the child in any way.
2. Interviews revealed that on 12/18/2023 adult 1 and staff 2 witnessed staff 1 jerk child 1 up by the arm to wipe his nose and the child started to cry.Plan of Correction: Discipline Policy was given to Staff member #1 to be reviewed again and signed.
Staff member #1 received a Correction Action.
More training and close attention from administrators will be given to insure that Staff member #1 remains compliant. Any further complaints will result in termination of employment.
Standard #: 8VAC20-780-410-1 Description: Based on interviews, the center failed to ensure staff did not use the forbidden actions of roughly handling a child.
Evidence:
1. Per interview with adult 1: On 12/18/2023 adult 1 and staff 2 witnessed staff 1 tell child 1(age 2 years) ?No we are not doing this.? and then jerk child 1 up by the arm to wipe his nose. The child started to cry.
2. Staff 3 and staff 4 stated staff 2 told them she witnessed staff 1 jerking child 1 up by his arm.Plan of Correction: Discipline Policy was given to Staff member #1 to be reviewed again and signed.
Staff member #1 received a Correction Action.
More training and close attention from administrators will be given to insure that Staff member #1 remains compliant. Any further complaints will result in termination of employment.
