Comments:
A monitoring inspection was initiated on 02/06/2024 and concluded on 02/06/2024 from 3:15 to 4:25 PM. There were 27 children present, ranging in ages from five to 10, with 3 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the three staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22.1-289.011-F
Description: Based on observation the center failed to post the most current inspection results in a conspicuous place on the licensed premises.

Evidence:

The inspection results dated 8/18/2023 was posted. The most current inspection conducted was dated 10/11/2023.

Plan of Correction: The correct inspection results were posted during the inspection. The results of this inspection will be posted upon receipt.

Standard #: 8VAC20-780-150-B
Description: Based on record review, the center failed to obtain documentation of a physical examination and immunization record that was signed by a physician, his/her designee, or an official of a local health department.

Evidence:

The record for child 3, start date 8/23/23, did not contain documentation of a physical examination and immunization record that was signed by a physician, his/her designee, or an official of a local health department.

Plan of Correction: The parents of child 3 will be contacted and advised to provide signed physical and immunization records. In the future physicals and immunizations will be checked for physician signatures upon receipt.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the center failed to ensure staff completed Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:

1. Staff 2's, start date 8/23/23, record did not contain documentation of completing the Virginia Department of Education-sponsored orientation course.
2. The director stated staff 2 said she completed the course but does not know how to get the certificate.

Plan of Correction: The director will assist staff 2 in figuring out how to get the certificate for completing the course. In the future the director will ensure all new staff provide the certificate for the record within the 90 days.
