1.There was not an attendance sheet for the 7 children observed in the childcare multipurpose room in the childcare building/center.

A portion, (approximately 2 ? feet long by 9 ? inches wide) of the top layer of the floor tile, in the center of restroom has been removed and the tile is broken, chipped, has sharp edges and it creates a tripping hazard.
