An unannounced monitoring inspection was conducted on 2/12/2024 from 10:30 am to 11:45 am. There were eight children in care, ages two and a half years to six years, in one group under the care of two staff. The Licensing Inspector reviewed three children?s records, three staff records, and other program records. The children were observed playing indoors.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.011-F Description: Based on observation, the center failed to post a document required by the Superintendent in a conspicuous place on the licensed premises.
EVIDENCE:
The result of the most recent inspection done on 8/28/2023 was not posted. It was emailed to the licensee on 8/21/2023.Plan of Correction: From now on, the most recent results from inspections (Violation Notices) will be posted as required.
Standard #: 8VAC20-780-130-A Description: Based on record review, the center failed to obtain immunization records for children prior to attending the center.
EVIDENCE:
1. Child one has a documented start date of 1/30/2024, and the immunization record was obtained on 2/2/2024.
2. Child 2 has a documented start date of 1/30/2024 and the immunization record was obtained on 2/12/2024.
3. Child 3 has a documented start date of 1/30/2024 and the immunization record was obtained on 2/12/2024.Plan of Correction: From now on, children will not be allowed to attend until the immunization record or conditional enrollment signed by the physician has been obtained.
Standard #: 8VAC20-780-60-A Description: Based on record review, the center failed to obtain two emergency contacts for each child.
EVIDENCE:
Children 2 and 3 have documented start dates of 1/30/2024. Each child has only one emergency contact listed.Plan of Correction: The director will obtain an additional emergency contact for each child and add it to the record.
Standard #: 8VAC20-780-580-L Description: Based on observation, text, interview, and record review, the center failed to obtain parental permission before the activity.
EVIDENCE:
On 2/6/2024, the LI attempted to do an inspection, but the lights in the center were off and the LI received a text from Staff 1 that the school was on a field trip.
Attendance records from 2/6/2024 show that children 1, 2, and 3 were present that day, and none of their records had permission from the parent to go on a field trip.
Staff 1 stated she forget to get the parents to give permission for the field trip.Plan of Correction: From now on the center will obtain written permission for field trips and leaving the center.
