Comments:
A monitoring inspection was conducted on February 6, 2024 with the center director and staff. There were 22 children present, ranging in ages from 3 years to 5 years with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Some information was sent electronically 2/14/2024.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of six staff records, the center failed to ensure that out of state background checks were conducted as required. Evidence: Staff 2 (date of employment 7/03/2023) indicated on the sworn disclosure statement dated 7/07/2023 that they had lived outside the state of Virginia within the past five years. There was no documentation to support that the required out of state child abuse and neglect registry check was submitted within 30 days as required. Staff 1 (date of employment 2/1/2024) indicated that they had lived outside the state of Virginia within the past five years. There was no documentation to support that the required out of state criminal history record check had been completed prior to employment.

Plan of Correction: The background checks will be submitted as soon as possible.

Standard #: 8VAC20-780-130-E
Description: Based on review of five children's records, the facility failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child 5 (date of birth 12/02/2019) did not have an updated immunization record since 9/22/2020.

Plan of Correction: The immunization record will be requested from the parent.

Standard #: 8VAC20-780-350-B-4
Description: Based on observation, the facility failed to maintain the ratio of 1 staff for every 10 children ages 3 years up to school age. Evidence: On 2/6/2024, there were 14 children with 2 staff. One of the staff left the room to get items from the dryer. The second staff was alone with 14 children for at least 5 minutes.

Plan of Correction: Staff will be reminded to maintain ratios at all times.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the facility failed to have a diaper disposal system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touched an exterior surface of the storage system during disposal. Evidence: the diaper disposal can did not have an operable foot pedal to open the lid.

Plan of Correction: The foot pedal will be fixed.

Description: Based on review of the emergency drill log, the facility failed to conduct a lockdown drill at least once annually. Evidence: there was no documentation of a lockdown drill since 12/07/2022.

Plan of Correction: A lockdown drill will be conducted annually.

Standard #: 8VAC20-780-560-J
Description: Based on observation, the facility failed to sanitize tables after each use for feeding. Evidence: the tables were not sanitized after afternoon snack. There were visible crumbs on the table while children began playing with play doh and kinetic sand.

Plan of Correction: Staff will be reminded to clean and sanitize tables after each use for feeding.
