Comments:
An unannounced inspection was initiated on 1/9/24 with the director in response to a self-report made by the center pertaining to an allegation of a forbidden action, rough handling of a child in care. Interviews were conducted, a classroom observation was conducted, 2 staff records were reviewed, and one child record was reviewed. There was insufficient evidence to support the allegation and no violations were cited based on the allegation. Other violations not related to the allegation were found during the inspection and can be viewed on the violation notice. An exit interview was conducted with the director on 3/4/24. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 3/11/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-160-A
Description: Based on review of two staff records, the center did not ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening at the time of employment. Evidence: The TB screening on record for Staff #2, with a re-employment date of 8/11/23, was dated 8/17/23.

Plan of Correction: Checklist created to ensure staff have everything for their file prior to starting.

Standard #: 8VAC20-780-180-C
Description: Based on review of two staff records and interview, the center did not ensure that staff who drive a vehicle transporting children disclose any moving traffic violation that occurred five years prior to or during employment or assignment as a driver. Evidence: Staff #1 and Staff #2 drive center buses but the staff records did not contain DMV driving records or a statement from staff regarding disclosure of moving traffic violations.

Plan of Correction: All driving records are in employee files.
