Comments:
A renewal inspection was conducted on March 4, 2024 from approximately 9:35 AM to 2:45 PM. There were 92 children present in the center, ranging in ages from infant to five years, with 24 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Additional documentation was requested and received on March 6, 2024 and March 12, 2024. A total of 14 children's medications and authorizations were reviewed. A total of 10 child records and 10 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names, list staff by positions only.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, the center did not ensure all hazardous substances such as cleaning materials insecticides and pesticides, were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In room #3, two containers of bleach and water and one container of Betco were unlocked in a cabinet on the wall. Children in the classroom are two years of age.
2. Staff in room #3, confirmed that the cabinet was not locked. Administrator #1 stated that she would look for a child proof lock for the cabinet.

Standard #: 8VAC20-780-510-P
Description: Based on a review of medications and authorization forms, the licensee did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization and that medications that are not picked up by the parent within 14 days are safely disposed of by the center.

Evidence:
The authorization forms for M# 8 and M# 9 had not been renewed and the medication was not returned or disposed of. The medication was on site.
