Comments:
A monitoring inspection was initiated on March 11, 2024, regarding a self-reported incident that occurred on March 7, 2024. The inspector was on site at the facility from 12:30pm to 2:00pm.
Physical space was inspected, and interviews were conducted. The information gathered during the inspection supports that the center was out of compliance with certain standards. Standard #: 8VAC20-780-40-E
Description: REPEAT VIOLATION
Based on staff interviews, the licensee did not ensure that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by the standards, resulting in a child being left unattended

Evidence:

Per interviews, the center's procedures for identifying where attending children are at all times include documenting the children's name, arrival and departure time as it occurs.

Staff #2 stated the normal procedures for the name to face check is to have the attendance sheet available in the classroom, record the children's names as they arrive, check name to face at the doorway in the classroom, and check name to face again on the playground.

Staff #2 stated that there was no name to face attendance sheet available in the classroom on the morning of 3/7/2024, and the children's names were not recorded as they arrived or departed the classroom.

Staff #3 confirmed the policy is to record the children's names on the attendance sheet as it occurs and to conduct a name to face check at the doorway of the classroom and again when entering the playground.

Staff #3 confirmed the name to face attendance sheet was not completed, and a name to face check was not conducted at the doorway of the classroom or when entering the playground by staff #1 and staff #2 on the morning of 3/7/2024.

Plan of Correction: In addition to electronicly
recording children's names
and arrival and departure times
as they occur, the center
procedures include additional
recording of this information on
a secondary paper record.
The staff have been retrained
on this procedure.
Daily paper attendance and

Standard #: 8VAC20-780-80-A
Description: Based on interviews with staff, for each group of children, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:

Staff #1 and staff #2, the supervising staff in the Preschool 3 classroom stated that on the morning of 3/7/2024, there wasn't an attendance sheet for the group documenting when children arrived or left the group. At approximately 10:10am, a staff member brought them an attendance sheet.

Description: Based on a review of staff records, the center did not ensure that one of three staff completed the Virginia Department of Education sponsored orientation course within 90 calendar days of employment.

Evidence:

Staff #2, first day of employment, 6/21/2023, did not have documentation on file of completing the Virginia Department of Education sponsored orientation course.

Description: REPEAT VIOLATION
Based on interviews with staff, the center did not ensure when staff are supervising children, they ensure their care, protection and guidance.

Evidence:

Staff #1 and staff #2 when supervising the Preschool 3 classroom on 3/7/2024, were not aware child #1 (3 years old) was left unattended in the classroom for approximately six minutes.

Staff #1 stated that on the morning of 3/7/2024, there wasn't an attendance sheet with the group and asked another staff member to bring them a copy of one.

Staff #1 stated that the classroom was "very chaotic" so they told staff #2 that they were taking 10 children outside, leaving staff #2 inside with five children.

Staff #2, after receiving the attendance sheet, realized that they didn't know who was actually present, so at approximately 10:13am, staff #2 took the remaining children outside to record the names of all of the children present.

At approximately 10:19am, staff #3 noticed child #1 alone in the Preschool 3 classroom.

Description: Based on interviews with staff, the center did not ensure that children under 10 years of age are always within actual sight and sound supervision of staff.

Evidence:

Staff #1 and staff #2, when supervising the Preschool 3 classroom on 3/7/2024, left child #1 (3 years old) unattended and out of sight/sound supervision in the classroom for approximately six minutes.

Staff #1 stated that the classroom was "very chaotic" so they told staff #2 that they were taking 10 children outside, leaving staff #2 inside with five children.

At approximately 10:13am, staff #2 took the remaining children outside to collect attendance, leaving child #1 behind.

At approximately 10:19am, staff #3 noticed child #1 was alone in the Preschool 3 classroom.
