Technical Assistance:
The inspector and the administration discussed the importance of spreading out the mulch on the playground.

Comments:
An unannounced monitoring inspection was conducted March 11, 2024 between the hours of 10:05am - 1:15pm. There were 22 children directly supervised by 4 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, posted documents, attendance, policies and the playground were inspected. Children were observed finishing snack, participating in free play, writing, putting on a puppet show and playing with the toy kitchen. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-70
Description: Based on record review, 5 out of 5 staff records did not contain all required information.
Evidence:
1) There is no documentation of emergency contact address available for Staff #1, Staff #2, Staff #3, Staff #4 and Staff #5.

Plan of Correction: 1. We have updated our Personal Emergency Contact Information Form to include addresses for each of the emergency contacts.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe, and operable condition.
Evidence:
1) Blind cords were observed within the reach of children hanging from the first window facing the parking lot, in the Monkey classroom.
2) The 4 seater seesaw on the playground outside was missing 1 of 4 large bolts on the surface of the structure.

Plan of Correction: 1. Blinds have been completely removed from the classroom.

2. We are waiting on a replacement bolt from the playground company. The 4 seater seesaw is no longer available for student use and caution tape has been placed around the equipment to prevent use.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place.
Evidence:
1) Cleaning sprays were observed in an unlocked upper cabinet in the Monkey classroom bathroom. The key was left inside the lock in an unlocked position. The cabinet was inaccessible to children.
2) Cleaning sprays were observed in an unlocked upper cabinet in the Zebra classroom bathroom. The key was left inside the lock in an unlocked position. The cabinet was inaccessible to children.

Plan of Correction: 1. Staff members in the Monkey Classroom were reminded that all hazardous substances must be in a locked cabinet out of reach from children. In addition, they have been reminded that the key may not remain in the lock even if the door is in the locked position.

2. Staff members in the Zebra Classroom were reminded that all hazardous substances must be in a locked cabinet out of reach from children. In addition, they have been reminded that the key may not remain in the lock even if the door is in the locked position.
