Comments:
A complaint investigation regarding supervision of children was initiated and concluded on 2/22/2024. The inspector arrived at 12:40 PM and departed at 2:01 PM. The investigation included review of documentation, observation and staff interviews. The inspector reviewed compliance in the areas of administration, physical plant and supervision.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:

Complaint related: Yes
Description: Based upon interviews and review of documentation, the facility has not ensured that they have notified the department?s representative as soon as practicable, but not to exceed two business days of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Evidence:
1. An accident report dated 8/8/2023, documented that seventeen-month-old child 1 climbed from a crib and fell, hitting his head on the floor causing a split in the skin and a raised bump.
2. Parent interview revealed that child 1 was taken to the doctor where the split on the forehead was closed using steri-strips.
3. The facility did not notify the inspector of the injury, nor was a ?Child Requiring Outside Medical Treatment Form? submitted to the Department.

Plan of Correction: The facility responded with the following:
In addition to notifying parents, we will make sure to send the form "child requiring outside medical treatment" to the licensing inspector/Department when needed.

Description: Based upon observation and interviews, the facility has not ensured that when staff are supervising children, they always ensure their care and protection.
Evidence:
1. On 8/8/2023, child 1, 17 months old, climbed out of a crib and fell to the floor, injuring his forehead . Staff interview revealed that the children had been placed in cribs while staff cleaned up after lunch was served. None of the staff interviewed stated that procedures were in place on August 8th to ensure adequate supervision of the children in cribs while cleaning up after lunch.
2. On 2/22/2024, the three staff on duty were observed cleaning up after lunch. Eight children over 12 months of age were observed in their cribs, six of whom were standing up in the cribs. An additional three children were on a rug in the center of the room. None of the three staff were directly supervising and/or interacting with the eleven children as all were busy with housekeeping and other duties.
3. Staff interview on 2/22 revealed that it was their procedure to put the children in the cribs while cleaning up. All staff were supposed to be cleaning up the room.

Plan of Correction: The facility responded with the following:
a. We have replaced the cribs for children over 12 months with cots, unless we have documentation that a child is not developmentally ready.
b. We put new procedures in place to have one staff member to clean up after lunch, while other staff members watch the children.

Standard #: 8VAC20-780-440-G
Complaint related: Yes
Description: Based upon observation, the facility has not ensured that cribs are used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat.
Evidence:
1. On 2/22/2024, ten of the eleven infants in care in the back infant room were identified by staff as being over 12 months of age. All of the infants were assigned a crib, not a cot or mat, for sleeping.
2. Staff interviewed revealed that children over 12 months of age sleep in cribs. Staff were not aware of any infants currently in care with developmental reasons why those over twelve months would be unable to sleep on a cot or mat.
3. A review of records revealed that only 1 of the 10 infants over 12 months of age included a documented developmental reason for sleeping in a crib and not on a cot/mat.

Plan of Correction: The facility responded with the following:
a. We have replaced the cribs for children over 12 months with cots, unless we have documentation that a child is not developmentally ready.
b. In addition to parents currently being able to state on the infant care form any concerns the school needs to be aware of, we added the wording "any developmental changes".
