Comments:
A self-reported monitoring inspection was initiated on 3/13/24 and concluded on 3/18/24. The inspector reviewed compliance in the area of staffing and supervision.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-340-F
Description: Based on a self report the center failed to ensure that children were in actual sight and sound supervision of staff as required.

Evidence:
At approximately 10:35am on 3/13/24 child #1, 3 year old, left the playground and went running towards the other playground.
Staff #4 saw the child running past her window and went out to retrieve him and give him back to his teachers.
Staff #1-#3 were unaware the child had left the playground.
Child #1 was out of sight and sound supervision of staff #1-#3 during this period of time.

Plan of Correction: 3/13/24-3/22/24 Staff will be provided with a summer safety training reviewing playground supervision, active supervision, and health & safety when outdoors. Including checking gates on the playground immediately upon going outside with their classroom.

3/13/24 - 3/19/24 All playground gates will be checked and fixed if child safety locks are no longer working properly.

3/13/24 ? Daily playground inspections will be conducted to ensure no gates are accessible by children, and noted on the Daily Playground Inspection List.
