Comments:
An unannounced renewal inspection was conducted with the director from 12:30 pm to 2:50 pm. Seventy three children were present with 14 staff supervising. The ages of the children present ranged from 6 months to 5 years of age.
The Licensing Inspector was able to observe children finishing lunch, handwashing and transition to their cots for naptime.
Medications for seven children were reviewed as well as 7 allergy care plans. Seven staff files were reviewed as well as 6 children's files. A sample size of 6 injuries/accident reports were reviewed today. Due to the rain, the playground was not inspected. The most recent fire inspection was dated 9/18/23 and the health inspection was dated 3/6/24. The areas of non compliance are outlined on the violation notice.

Violations:
Standard #: 22.1-289.035-B-2
Description: The standard requires fingerprint background checks prior to employment.
Staff A was employed on 3/9/24, however the fingerprint background was dated 3/20/23.

Plan of Correction: I ran the timesheet and included January 1, 2023 up and until April 3, 2024. Although staff A's orientation was dated 3/9/23, the first working day in a classroom with children was after her fingerprint background was sent. Below is a screenshot of the timesheet for March 2023. I will attach a copy of this in the file for future reference.

Standard #: 8VAC20-780-240-B
Description: The standard requires orientation training prior to working alone with children and no later than 7 days of the date of assuming job responsibilities.

There was no orientation training documented for Staff A who was hired on 3/9/23.

Plan of Correction: Staff A was given orientation on 3/9/23, the orientation has been added to the new orientation form and has been added to the employee file.

Standard #: 8VAC20-780-280-B
Description: The standard requires all hazardous substances be kept locked.

1. In an unlocked closet in room 103 housed hazardous substances such as disinfectants, sanitizing solutions and fragrance sprays.
2. Unlocked on tne the top shelf in room 109 house a sanitizer and water solution in a spray bottle.
3. In an unlocked closet in room 133 housed a sanitizer and water solution in a spray bottle.

Plan of Correction: 1. Staff in room 103 were reminded of school and state policies to ensure that all hazardous substances are always locked away after cleaning the tables. Administration conducts 15-minute checks throughout the day to ensure ongoing compliance.
2. Staff in room 109 were reminded of school and state policies to ensure that all hazardous substances are always locked away after cleaning tables. Administration conducts 15-minute checks throughout the day to ensure ongoing compliance.
3. The lock in room 133 will be repaired and the staff have been reminded to ensure that the door is always secured until it is repaired. An extra child lock has been installed until the handyman gets here.
