Standard #: 8VAC20-780-140-A Description: Based on record review and interview the center did not ensure that each child have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
Evidence:
1.Child #1 (enrollment date 8/26/2023) did not have a physical examination in their record.

2.The program director confirmed that child #1?s record did not contain a physical examination.Plan of Correction: The director will contact the child's parent to obtain a current physical exam and will ensure it is placed in the child's file.
Standard #: 8VAC20-780-245-A Description: Based on record review and interview the center did not ensure that all staff complete annually a minimum of 16 hours of training appropriate to the age of children in care.
Evidence:
1.Staff #1 had 6 of 16 required hours of training in 2023.
2.The program director confirmed staff #1 did not have the required 16 total hours of required training in 2023.Plan of Correction: The director believes the staff member has ad the training for 2023. She will look through old files to try and locate additional trainings.
