Comments:
A complaint inspection was initiated on 03/07/2024 and concluded on 04/04/2024. A complaint incident was received by the department regarding allegations in the areas of staff-to-children ratio and group size requirements, supervision of children and operational responsibilities. Observations were made, interviews were completed, and documentation was reviewed. The preponderance of evidence gathered supports violations of the standards. Areas of noncompliance can be found in the violation notice. Areas of noncompliance not related to the complaint but identified during the course of the investigation can also be found on the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Description: Based on record review, the center did not ensure that each child record contains all required information.
Evidence:
1) There was no emergency contact information on file for Child #3.

Plan of Correction: One new contact has been added.

Complaint related: No
Description: Based on record review and interviews, the center failed to have the required staff-to children ratios whenever children were in care.
Evidence:
1) According to the facility's Child Supervision Record (CSR) document, on 02/28/2024 the Discovery Preschool classroom (2 year olds) was out of ratio from 9:15am until Staff #7 arrived at 9:20am. 17 children were present at 9:15am with Staff #3 and Staff #10.
2) Staff #6 confirmed that the classroom was out of ratio during the time mentioned.

Plan of Correction: We have initiated a new code that will be called when any classroom is 1 child away from being out of ratio. (Via walkie talkie)

Description: Based on record review and interviews, the center failed to have the required staff-to children ratios whenever children were in care.
Evidence:
1) According to the facility's Child Supervision Record (CSR) document, on 03/07/2024 the Pre-Kindergarten classroom (4-5 year olds) was out of ratio from 8:25am until 8:42am. 11 children were present at 8:25am with Staff #8.
2) According to the facility's Child Supervision Record (CSR) document and child record review, on 03/05/2024 Child #3 (Age 2) was in the Preschool classroom (3-4 year olds) from 9:20am ? 11:50am. Child #3 does not meet the age requirement to be at a 10:1 ratio. At 9:20am Child #3 was the 17th child in the Preschool classroom with Staff #9 and Staff #2.
3) Staff #6 confirmed that the classrooms were out of ratio during the times mentioned.

Plan of Correction: We have initiated a new code that will be called via walkie talkie when any classroom is 1 child away from being out of ratio. Instead of moving a younger child up, we will move them back so that the ratio will not change.
