Comments:
An unannounced monitoring inspection was conducted on 04/05/2024. There were 106 children present and 14 staff. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed children and staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and interview it was determined the center did not ensure to have a physical examination by or under the direction of a physician before the child?s attendance or
within 30 days after the first day of attendance.
Evidence:
1.Child #1 (date of enrollment 2/5/24) did not have a physical examination in their record.
2.The program director confirmed that the physical examination for child #1 was not in the child?s record.

Plan of Correction: Family is from out-of-state. Director will contact parents and obtain a physical for the child.

Standard #: 8VAC20-780-290-A-3
Description: Based on observation the center did not ensure that electrical outlets have protective covers that are of a size that cannot be swallowed by children.

Evidence:
In the toddler classroom, there was an electrical outlet by the radio that was missing a protective cover.

Plan of Correction: Electrical outlet was covered during the inspection.

Standard #: 8VAC20-780-350-B-3
Description: Based on observation and interview the center did not ensure that the required staff-to-children ratios were followed whenever children are in care.
Evidence:
1. There were 2 staff and 20 children in the three-year-old classroom.
2. .The youngest child in the classroom was two, requiring a ratio of 1:8
3. Staff confirmed that the youngest child was two years old and therefore the ratio requirement was 1:8.

Plan of Correction: An assessment and transition form will be completed prior to twos visiting and then moving into the three year-old room.
