An unannounced monitoring inspection was conducted on 04/02/2024 in response to a self-reported incident. There was a total of 15 children and 5 staff members present during the inspection. The inspector reviewed video footage, interviewed 1 staff members and reviewed a total of 1 child record, 4 staff records and 1 injury record during the investigation. The inspector arrived at 11:40 am and departed at 1:05 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard #: 8VAC20-780-60-A Description: Based on record review, the facility failed to ensure that the separate record for each enrolled child contains all of the elements as required by the standards.
Evidence: The record for Child 1 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there were no addresses provided for either designated person).Plan of Correction: Will call the parent and get the missing information added to the record.
Standard #: 8VAC20-780-550-P Description: Based on document review, the facility failed to ensure that the written record of children?s injuries contain all of the elements as required by the standards.
Evidence: The injury record for Child 1 did not contain the following information: the time when parents were notified; and staff and parent signature or two staff signatures (there was only one staff members signature on the injury record).Plan of Correction: Will make sure that all required information is on the injury record.
