The licensing inspector conducted an unannounced complaint inspection on 04/11/2024 In response to a complaint that was received in the licensing regarding allegations in the area of Programs and Emergencies. Interviews were conducted with staff, records reviewed and observations of the program were made relating to the allegation. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Complaint related: No Description: REPEAT VIOLATION: Based on a record review and interview the center did not ensure all staff have the required repeat background checks.
Evidence:
1. Staff#2, who has worked alone with the children in care, did not have documentation of a central registry check within 5 years of the last one.
2. The last central registry check for Staff #2 was dated 2018.
3. The director confirmed staff #2 did not have a current central registry check.
4. Both staff #1 and #2 confirmed Staff#2 has been left alone with children.Plan of Correction: Staff #2's central registry has been sent out. The director will contact the Office of Background Check Investigations to check on its status.
Standard #: 8VAC20-780-550-P Complaint related: Yes Description: Based on a record review and interview the center did not maintain written record of children's serious and minor injuries.
Evidence:
1. Child #1 sustained an injury to her lip while in care on 04/01/2024. The center did not have written record of the child's injury.
2. The director confirmed the child did sustain an injury to the lip and the center did not have documentation of the injury.Plan of Correction: The injury will be documented in the center's records. n the future the center will have the parent sign before taking the incident report.
