Comments:
An unannounced monitoring inspection was started in the center on April 16, 2024 from 3:45 PM until 6:00 PM and concluded on April 29, 2024. At the time of entrance, 19 children were in care with 3 staff members. The sample size consisted of 5 children?s records and 5 staff records. Children were observed eating snack, washing hands and preparing for their afternoon routine and playing with kitchen toys.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review and staff interviews, the licensee did not ensure to obtain documentation that each child has received a physical examination by or under the direction of a physician before the child's attendance; or within 30 days after the first day of attendance.

Evidence: The record for Child #3 did not include a physical and the first date of attendance was 2-13-2024.

Plan of Correction: The #3 child had a physical but it was missing one page of the document. Complete document placed in child's file.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review and provider interview the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: The following groups of children did not have adequate attendance sheets
1. Staff #3 verified there was not attendance sheet that marked the arrival or departure for infants as it occurred.
2. Staff #1 verified there was not an attendance sheet for the two-year-old room or for the 3- and 4-year-old room that marked the departure of children in care as it occurred.

Plan of Correction: Noted! I will be sure to wash my hands

Standard #: 8VAC20-780-330-B
Description: Based on observation and staff interviews, the licensee did not ensure where playground equipment is provided, there shall be under equipment with moving parts or climbing apparatus enough resilient surfacing to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.

Evidence: On the playground there was a merry-go-cycle and there was not resilient surfacing available.

Plan of Correction: The same equipment has been observed for 6 years and never considered large enough for a barrier. Equipment to be removed.

Standard #: 8VAC20-780-500-A
Description: Based on observation and staff interviews, the licensee did not ensure to wash their hands with soap and running water after coming in contact with body fluids.

Evidence: Staff #1 was observed wiping 2 children?s nose and did not wash their hands after.

Standard #: 8VAC20-780-500-B
Description: Based on observation and staff interviews, the licensee did not ensure that staff use a nonabsorbent surface for diapering or changing as required for children younger than three years. The diapering surface for children younger than 3 shall be a changing table or countertop designated for changing. the required procedures were followed for diapering.

Evidence:
Staff #2 was observed changing the diapers of 2 children, ages 2, (verified by Center Director) and each child stood in the bathroom while Staff #2 changed their diaper while they were standing up

Plan of Correction: All staff will use changing table for 2 year olds

Standard #: 8VAC20-780-560-F
Description: Based on observation and staff interviews, the licensee did not ensure to follow the recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) when they choose to provide meals and snacks.

Evidence:
1.The menu posted was for the January 2024 and there was not a meu available for review for the current week.
2.The snack observed served did not meet the USDA guidelines as it only included fruit snacks and two food groups were not served as required.

Plan of Correction: Noted! All food guidelines will be followed
