Comments:
A monitoring inspection was conducted on April 24, 2024, from 9:25 AM until approximately 10:55 AM. There were 14 children present, ranging in ages from 2 years to 5 years, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on a review of five child records, the center did not ensure that each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Evidence: The record for child #1 did not contain a physical. Child #1?s date of enrollment was 12/11/2023.

Plan of Correction: Will place physical in folder.

Standard #: 8VAC20-780-550-E
Description: Based on record review, the center failed to ensure that shelter in place procedures shall be practiced a minimum of twice per year.

Evidence:
1. The drill log did not have documentation of practiced shelter in place procedures from May 2023 ? April 2024.
2. Staff #2 stated that shelter in place drills were conducted but were not documented.

Plan of Correction: Will label what drills are being done.
