Comments:
An unannounced monitoring inspection was conducted on 4/23/24 from 9:30am to 11:00am with the Director. There were 28 children in care, ranging in age from six-months to five-years-old, supervised by six staff. The children were observed participating in circle time, dancing, learning about the rain forest, and doing addition problems as a group. Five child records and three staff records were reviewed. The Center has 13 staff with current certification in CPR and First Aid, and 13 staff trained in Daily Health Observation. The Director reported that there was no medication on site, but the center has five staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, allergy list, and emergency drill log were reviewed. The most recent Fire Inspection on file was dated 8/4/23 and the most recent Health Inspection on file was dated 9/22/23. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 5/1/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on review of five child records, the center did not obtain documentation that each child had a physical examination by a physician within 30 days after the first day of attendance. Evidence: The record for Child D, with a start date of 2/22/24, did not contain documentation of a physical.

Plan of Correction: Parent is requested for an updated record. Child is having doctor's appointment on 5/5/24.

Standard #: 8VAC20-780-80-A
Description: REPEAT VIOLATION Based on observation, the center did not maintain a record of daily attendance for each group of children that documented the arrival and departure of each child in care as it occurred. Evidence: On 4/23/24, at approximately 9:33am, the attendance in the toddler classroom contained documentation of three children present. There were two children present in the classroom at that time. At approximately 9:37am, the attendance in the infant classroom contained documentation of two children present. There were three children present in the classroom at that time.

Plan of Correction: Records were immediately corrected and teachers are given the guidelines for future.

Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: On 4/23/24, on the large playground, there were two large screws observed protruding from the ground where a railroad tie was previously in place. The screws posed an impalement and trip hazard. The screws were approximately one inch wide; one protruded from the ground approximately 1.5 inches high, the other protruded from the ground approximately two inches high.

Plan of Correction: The screws are completely hammered in the ground and the area is completely secured and safe.
