This monitoring inspection was initiated by licensing staff and concluded on 04/24/2024, between the hours of 9:30 am to 1 pm. There were 51 children present, ranging from infant to preschool-age, with 8 classroom staff supervising, across 6 groups. The children were engaged in the following: circle time, freeplay, centers within the classroom, clean-up, diapering, hand washing, music, lunch, nap time, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children files, and 11 staff files were reviewed.
Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Standard #: 8VAC20-780-160-A Description: Based on review, 1 of 11 staff files indicated that each staff member did not submit documentation of a negative tuberculosis screening(TB) within the last 30 calendar days to the date of employment, signed by a physician (or physician's designee, or an official of the local health department) and prior to coming into contact with children.
Evidence -
1. On the date of inspection (04/24/2024), TB documentation was obtained by Staff #10 after their date of employment - 03/25/2024.
2. TB documentation was dated - 03/27/2024.Plan of Correction: TB documentation will be obtain by Staff #10 and placed on file.
Standard #: 8VAC20-780-60-A Description: Based on review, 1 of 5 children files indicated that not all of the required information was contained within each child's record.
Evidence -
1. On the date of inspection (04/24/2024) the proof of identification was not available for review for Child #3.
2. The start date for Child #3 was listed as 03/25/2024.Plan of Correction: Proof of Identification will be obtained from the child's parent.
